ALL copayments

ALL copayments, deductibles, and account balances are due on arrival for your visit and PRIOR to seeing the healthcare provider.

Copayments cannot be waived. Copays are contractual agreements between you and your insurance company, as well as between Southside Diabetes and your insurance company. If we do not collect your copay, you and SSD are both in breach of contract with your insurance company. Your insurance company is then entitled to cancel your coverage, drop our contract, and you will be responsible for the cost of the entire visit.

We verify your insurance before each visit. If inactive or not used, you’ll be treated as a Self-Pay patient. Prices below apply to cash payments only.

Financial Agreement

A signed Financial Agreement (Assignment of Benefits) is required from all patients prior to their scheduled appointment. This document also authorizes Southside Diabetes LLC to securely maintain your credit card information on file.

It outlines the responsibilities of both the patient and the practice, confirming that services rendered will be billed appropriately under your insurance plan or self-pay terms.

Billing

Our office submits insurance claims as a courtesy; however, payment responsibility ultimately rests with you. We recommend reviewing your insurance coverage and verifying your benefits directly with your provider.
Once your insurance has processed the claim, any remaining balance is your responsibility and should be paid promptly.

Copayments

All copayments, deductibles, and outstanding balances are due upon arrival and must be paid before seeing your healthcare provider.
Copayments cannot be waived, they are contractual obligations between you, your insurance carrier, and Southside Diabetes. Failure to collect copays places both you and our practice in violation of these agreements. In such cases, your insurance provider may cancel coverage, terminate our contract, and hold you responsible for the full cost of services rendered.

Cancellations

Same-day cancellations and no-shows will result in a $50 fee, which must be paid prior to rescheduling. Please review our No-Show & HIPAA Policy [here]; printed copies are available upon request.

We understand that unexpected circumstances arise. However, missed appointments leave a 20–40 minute gap in our schedule, resulting in lost productivity and revenue for SSD. Because the clinic continues operating even during missed visits, this fee helps recover a small portion of the lost time and resources.
