Telemedicine is an evidence-based option to help you successfully bridge the gap between the level of care you have and the health you want.
Diabetes Device Portal Links
If you currently use a diabetes device, such as an insulin pump or sensor (continuous glucose monitor), please click the appropriate link below to create an account if you have not done so already. Write down your username and password; you will need them to pair with our clinic for select devices. We are not able to link with glucometers.

On the day of your telehealth visit:
- You will will receive a phone call from our medical assistant. She will confirm your medications and any additional information you entered via the Patient Portal.
- You will then receive a text message from Doximity when your provider is ready to start the visit. The link is secure and HIPAA-compliant. No additional software or downloads are needed.
- If you have any difficulty joining the visit, or need the telehealth link resent, please contact the office 757-659-9903 ahead of your scheduled appointment time.
