Telemedicine is an evidence-based option to help you successfully bridge the gap between the level of care you have and the health you want.

Diabetes Device Portal Links

If you currently use a diabetes device, such as an insulin pump or sensor (continuous glucose monitor), please click the appropriate link below to create an account if you have not done so already. Write down your username and password; you will need them to pair with our clinic for select devices. We are not able to link with glucometers.

On the day of your telehealth appointment:

  • You will join the visit at least 15 minutes early using the text or email link sent from athenahealth. The link is secure and HIPAA-compliant. No additional software or downloads are needed.
  • You will start the visit with our medical assistant. She will confirm your medications and any additional information you entered via the Patient Portal.
  • You will remain in the visit as the medical assistant exits. Your healthcare provider will then join the appointment.
