1. Appointment Policy

All appointments must be scheduled in advance. Same-day cancellations and no-shows will incur a $50 fee, payable prior to rescheduling. This ensures fair access to care for all patients and helps maintain an efficient schedule.

2. Insurance Verification

Insurance coverage is verified prior to every visit. If your insurance is inactive or cannot be confirmed, you will be classified as Self-Pay and must pay the self-pay rate before being seen.

3. Financial Agreement

A signed Financial Agreement (Assignment of Benefits) is required for all patients prior to their appointment. This document authorizes Southside Diabetes LLC to securely keep your credit card on file and bill insurance on your behalf. Patients are financially responsible for all charges not covered by insurance.

4. Billing & Payments

All copayments, deductibles, and outstanding balances are due upon arrival. Copays cannot be waived, as they are contractual obligations between you, your insurance company, and Southside Diabetes.

Unpaid balances must be resolved promptly. Accounts that remain unpaid after reasonable notice may be sent to collections.

5. Fees & Refunds

The $50 no-show or same-day cancellation fee is non-refundable. Any other refund requests will be reviewed on a case-by-case basis by clinic management.
