The team at Southside Diabetes is pleased to offer remote patient monitoring (RPM) in partnership with Optimize Health.
Research shows that RPM can help keep patients out of the hospital by proactively monitoring such conditions as diabetes, high blood pressure, and heart failure. We will monitor such metrics as:
- Blood pressure
- Blood sugar
- Weight
- Oxygenation
The team at Optimize Health will monitor your numbers, contact you directly if you’re missing readings, or call to see how you’re doing if your readings have changed. For example, if your blood pressure is higher than normal, the team of registered nurses (RNs) at Optimize Health will make sure you’ve taken your medication and alert our office if you need a refill. The RNs will also notify SSD if your health status has changed so we can call you and/or schedule an office visit to update your treatment plan.
Optimize Health will ship the RPM equipment (such as blood pressure machine and scale) directly to your house, then call to help you set it up. You will complete your measurements each morning from the comfort of your own home. The equipment and monitoring you receive will be based on your individual medical conditions.
